The WISe Care Coordinator is a member of the Wraparound with Intensive Services (WISe) team, who is specially trained to coordinate and facilitate the WISe process for an individual family, and provide advanced care coordination for the phases and activities of WISe. The Care Coordinator typically facilitates the Child and Family Team (CFT) meetings, and is ultimately responsible for leading the team through the phases and activities of WISe, both during and outside of the CFT meetings. The Care Coordinator contributes knowledge and skills related to making sure that the team process honors each member’s role, responsibility, and perspective.

Work schedule is usually Monday – Friday, 8:00am – 5:00pm. Occasionally meetings may be scheduled before or after these hours. This position also participates in the on-call rotation after a 6-month onboarding period.

Qualifications

  • Education: Bachelor’s Degree in Social Work, Psychology, Counseling or other relevant field required.
  • Experience: Minimum of one year of experience working with families and children in a therapeutic or educational capacity.
  • Licenses/Certificates/Registration: Application for registration as an Agency Affiliated Counselor in the State of Washington within 60 days of employment and receipt of registration with 120 days of employment; current Washington State driver’s license and proof of automobile liability insurance coverage; current First Aid/CPR card, or obtained within 45 days of hire. NOTE: FBI fingerprints required if you have lived in Washington State less than the last 3 consecutive years.
  • Knowledge/Skills/Abilities Required: Knowledge of: Federal, State and County social services regulations and systems; State mental health and special education requirements; psychological principals in learning and their application to treatment stages; advanced evaluative techniques of personality, academic behavior, and cognitive domains. Ability to: work independently; exercise independent judgement and decision-making regarding the safety and learning capabilities of individuals and family members served; work as part of a team; utilize the DSM V; represent YVFWC Behavioral Health Services in contact with both State and County agencies; and synthesize and formulate the assessment data into a comprehensive treatment summary. Candidates with bilingual English/Spanish skills are preferred.
